Determine
your Contract Status
You can download forms for Construction or Procurement and professional
Services contracts below. You can also download the Certification Check
List below and recent changes and modifications that affect this process.
Re-certification Requirements
Certification is an annual process. After your initial certification,
if you want to be re-certified, you will need to reapply; however, the
re-certification process is not as involved as long as you file your forms
annually or at least less than six months after your certification expiration
ends.
Within
six months after expiration date, you will need:
- Cover letter requesting re-certification
- Updated financials (notarized bank account and verification of financial
statements)
- List of past projects (list, type, location and dollar amount)
- Significant changes (of company organization, structure, activity,
or ownership)
If you are requesting re-certification six months after your certification
has expired, you must formally re-apply.
